The elevator entrance bells ding and a second wave of monsters are unleashed. Screaming and buckets of blood splatter our view. The facility is in a complete chaos of carnage as the workers are overwhelmed and slaughtered by the Army of Nightmares.


Our view is focused on one elevator stall, the door opens revealing Hell Lord.


A lone guard is near an elevator stall when the door opens. The Alien Beast pounces on the guard's face. The Alien Beast [summoned by an alien egg (fan theory)] is a facehugger-like creature that appears to exude acidic yellow blood when shot. This monster is likely an homage to the "facehugger" lifecycle stage of the Xenomorph in the Alien franchise, originally designed by H. R. Giger.


Another elevator door opens: The Dolls [summoned by a similar mask seen in the cellar (fan theory)] - humanoid figures wearing doll masks and black clothing. The Dolls are a family of cruel and sadistic killers, comprised of an older woman in a corporate suit, an older man in a business suit, a blonde pigtailed girl in a black dress, and a hooded boy.


We cut to another hall in the facility, a Wraith is soaring through, terrorizing workers as they scramble.


A (possible) Deadite [summoned by a Necronomicon verse tape recording (fan theory)] grabs a worker and throws him over the railing. Before hitting the floor, the Giant Snake catches the worker in its mouth and eats him.


The Giant Snake [summoned by a snake skin (fan theory)] appears to be a mixture of cobra, rattlesnake and anaconda/python, but substantially larger proportions.


Hell Lord can be seen on a video monitor about to torture a facility worker chained upside down. Hell Lord is accompanied by two figures dressed in similar clothing, hinting that they are either his minions or partners. One is bald and has his face partly obscured by clothing. The other is a female with sinister eyes, and her mouth is similarly covered.


The Mecha Scorpion, or Killer Robot, is climbing down a stairwell in hot pursuit of a worker who is tumbling down the stairs. The killer machine swings its buzz saw in action. The Mecha Scorpion [summoned by a rusty buzz saw (fan theory)] is an enormous yellow spider-like robot with four fast pneumatic legs.

It's arms; two small ones in front and a long one in back, hold loud, squealing buzz-saws. The smaller arms are capable of shooting out a pair of grappling hooks to snag and drag humans closer to the robot, allowing it to deliver a killing blow with its buzz-saws.


The Doctors [summoned by surgical equipment (fan theory)] are seen in a camera view preparing to perform surgery on a worker who is begging them to not cut him. One of the doctors is seen holding up a bloody scalpel. The Doctors' faces are mutilated and their surgical clothing is stitched to their flesh. They torture and kill by performing barbaric vivisections with scalpels on conscious victims.


One of the Reanimated crawls up to a camera and curiously looks at it. The Reanimated [summoned by a serum (fan theory)] are depicted as living corpses, similar to zombies, but with their extremities, head and torso rearranged in twisted ways. They have the ability to crawl on walls like spiders and appear to be made out of bodies that have been chopped up and then crudely put back together. The main disfigurement being their heads placed upside down.


While one of her co-workers is being attacked behind her, a woman in the Demolition department decides to spare herself from terror and agony, so she commits suicide by blowing her brains out.


A bank of video monitors show various creatures running loose in the facility. The Dolls can be seen in a camera view angle binding and gagging facility workers together with tape before dousing them in gasoline and setting them on fire. The Huron brutally scalps and kills a facility worker (not shown).


A Suffocator is seen suffocating a person with a plastic bag. The Suffocators [summoned by a plastic bag (fan theory)] are muscular men with plastic bags on their heads, apparently they are not in the supernatural spectrum of monster, but rather the category of evil human beings.

The Dismemberment Goblins are taking a golf cart for a joy ride, with the green one holding the head of a facility worker in its hand. The Mummy is trying to grab a worker as he runs by. The Mummy [summoned by Egyptian dagger (fan theory)] is shown as a re-animated semi-rotting, mummified corpse that uses a long curved knife or dagger.


Reptilius [summoned by a reptile claw (fan theory)] stalks a woman from behind, about to attack and kill her. Little is known about this monster other than it's shown physical characteristics - a reptile/lizard-like creature that stands upright like a human.


The Man in Transparent Tarp is wandering around the public bathrooms, presumably looking for victims. The Man in Transparent Tarp [summoned by transparent tarp seen in cellar (fan theory)] is a skinless creature with exposed organs wearing a see-through tarp all over its body. It is shown holding a sledge hammer..

The Mutants [summoned by tampering with the Gas Mask and Tonics] hold a victim down while another vomits acid into the captive's mouth. The Mutants appear as distinctly humanoid beings in contamination suits, with the added scarification and distorted features characteristic of other common mutants in fiction.


The Angry Molesting Tree is seen using its roots to drag an unconscious worker off screen. A creature or victim is seen crawling on the floor with it's legs amputated. The Twins slowly follow someone that has been set on fire. They themselves may have been the ones who set the person on fire (indicating a possible pyrokinetic ability between the two sisters) or another monster set someone on fire.


Marty and Dana are still cornered in the room, keeping low. Truman arms himself as he calls for . . .

TRUMAN: Lead officer Truman to security command. Requesting immediate reinforcements. Code black. I repeat, code black. Where the fuck are you guys?


Lin, Hadley, and Sitterson are frantically trying to regain control as the video monitors show various monsters continuing their attacks on the workers. Ronald the Intern can be seen holding up a sign to one of the cameras in the main security room, claiming that the Dragonbat has his scent and he needs help fast. Video monitor highlights:


The Sugarplum Fairy is dancing in a room that is splattered with blood. She is surrounded by several mutilated victims.


The witch steals the soul of a woman attempting to run from her. A Vampire [summoned by: a bottle of Vampire Breath] slowly creeps out of the darkness behind a facility worker and grabs her by the face. It is almost identical to Count Orloc, the vampire in the old silent film Nosferatu. The Sasquatch/Wendigo/Yeti [summoned by a jar of Eau de Sasquatch] beats a member of the Security Force to death in the Facility Locker Rooms. The creature resembles a large, hairy humanoid, akin to the characteristic images in popular culture.


The Jack O'Lantern [summoned by lighting the three small Jack O' Lantern candles (fan theory)] is seen burning people with fire breath. The Jack O' Lantern is a tall man with a carved pumpkin for a head. He seems to wear a 17th century gentleman's coat with a frilled cravat.



HADLEY: Sector 12, down. Sector 8, down. Sector 3, down.

SITTERSON: Why aren't the defenses working? Where's the fucking gas?

HADLEY: Something chewed through the connections in the utility shaft.

SITTERSON: Something which?


HADLEY: Something scary!

The Clown is seen mocking Hadley from his monitor. The Clown [summoned by the fortune teller machine] has no unusual physical features, bearing the stereotypical appearance of a clown: poofy red hair, red nose, colorful makeup, and baggy, multi-colored clothes with frilly trim. He dispatches victims by stabbing them with a long, curved knife while giggling and laughing hysterically.


The control room blacks out, then back-up lights kick in. Something huge and ferocious is bashing at the massive steel vault door leading to the control room. A third elevator entrance bell dings and a third wave of monsters are unleashed (implied).


The Dragonbat crashes through the control room Marty and Dana are hiding in. The Dragonbat [summoned by: unknown] is a giant bat with red scales on each side and a mouth that is broken into three parts. This creature is one of the most powerful and deadly of all the monsters. It shrieks and hisses at Marty and Dana, they race out of the room and are now in the midst of the creature attacks.
